Role: Facilitates the development and implementation of effective community living environments for people who have disabilities so they can achieve their personal goals.


Essential Functions & Responsibilities

  1. Recruits and assists up to 20 people who have a disability with: developing, understanding progress, and achieving personal goals; identifying and sustaining support networks; and remaining healthy and safe in a community living setting.
  2. Coordinates communication with people who have a disability, their family members, guardians, coworkers and other professionals in a manner that maintains high quality of care and customer service; communications reflect all legal and ethical requirements.
  3. Successfully recruits, trains, and authorizes Host Home Providers; ensures a sufficient pool of these providers exists to promote choice in placement for people who select HSI as their Supportive Living Arrangement Provider.
  4. Oversees independent contractor relationships with Host Home Providers and provides constructive feedback when appropriate.
  5. Monitors the effectiveness of services provided and assures people, families, guardians, the State of Nevada, and other customers meet internal quality standards.
  6. Develops professional business relationships internally and externally to facilitate meeting customer needs and departmental/organizational goals.
  7. Completes documentation and paperwork required to ensure people obtain and maintain: entitlement benefits, contracted services, community resources, and basic needs (including rep payee).
  8. Maintain appropriate boundaries with people receiving services and their family members and follow organizational policy and procedure to ensure the person receiving services receives the best care possible.



Supervisory Responsibilities:

  • Document and monitor performance of assistant case manager when working on a shared case.

Physical Requirements and Demands:

  • Regularly required to speak, listen and communicate.
  • Regularly is required to stand, walk, sit, grasp, handle objects or feel objects, and reach with hands and arms.
  • Occasionally pulls, bends, stoops, and crouches.
  • Specific vision abilities required by this job include close vision and ability to adjust focus.
  • Lifts or carries 50 pounds unassisted to assist with lifting and transferring of persons who use wheelchairs and other mobility assisted devices.

  • Multi-line telephone, calculator, PC, cellular phone and fax/copier.
  • Motor vehicle

Education and/or Experience:

  • Bachelor's Degree in a Health and Human Services related field. MA or BSN Preferred.
  • At least one year of Case Management experience and one year working with adults who have developmental disabilities, physical disabilities, and/or mental health disabilities.
  • Must meet definitions of Qualified Intellectual Disabilities Practitioner (QIDP) as per governing bodies.
  • Candidates without case management experience may be considered for an Assistant Case Manager role.



Certificates, Licenses, Registrations:

  • Must have a current CPR/FA certification or attend company CPR training within 30 days.
  • Must possess and maintain current NV Drivers License, automobile insurance and clean driving record.



  • Ability to meet background checks requirements of the Organization.
  • Intermediate use of computer applications such as Excel, Word, and other PC based programs.
  • Tolerance of ambiguity and ability to perform essential functions with minimal supervision.
  • Ability to multi-task effectively and be flexible amidst frequent changes to work flow.
  • Excellent organizational skills to appropriately manage large amounts of information.
  • High level of interpersonal skills to handle sensitive and confidential situations appropriately.
  • Commitment to person-centered, culturally competent philosophical base.
  • Safe and reliable vehicle to transport people receiving services.
