Role Summary:  Using discretion and independent judgment this position oversees the delivery high quality services for people who have an intellectual disability, advocating for people served and providing leadership and supervision to direct service staff. The AQIDP is a key ambassador of the iChoose department culture, assisting with implementing systems and ensuring that day-to-day operations are aligned with the iChoose vision. 

 

Essential Functions & Responsibilities

Essential Functions:

  • Assess individuals' skill level and analyze data for selection and individualization of plans.
  • Train staff members on data collection and data entry of assessed skills.
  • Actively monitor staff measurement of skills during activity periods, assess progress and effectiveness of supports provided to people receiving services, and ensure staff are consistent with measurement and data collection during activity periods. This is done through but not limited to:
    • On-site observations
    • Reviews of services and documentation
    • Discussion with people served
    • Review of FYI's and incident reports, notifying chain of command as appropriate
  • Regular review of data, and revision to support plans based on individual needs or performance.
  • Attend Annual Integrated Team meeting for all service recipients and Quarterly Integrated Team meetings as needed. Meet with staff and service recipient prior to the meeting, review data and discuss challenges and progress in all assessment areas. Ensure adequate environmental supports and assistive devices are present to promote independence.
  • Ensure that service recipients' daily activities are in alignment with Medicaid, State and Federal Regulations as well as Division policies.
  • Assist people receiving services and family members, guardians, or other support persons with understanding the progress achieved towards stated action/service plan goals and the services we will provide to help with attainment of these goals. Communicate progress and other important information (health issues, incidents, social behaviors, etc) regularly to ensure continuity of care and ease coordination of services.
  • Observe, coach, and provide feedback on services provided by direct and support service staff.
  • Participate in training and learning opportunities for professional development, and integrate best practices and emerging practices into programming. 
  • Assist with Teaching Pre-Employment transition Services as needed.
  • Continually assessing the environment for safety concerns, problem solving process related safety concerns and reporting concerns to appropriate management.
  • Assist with crisis management utilizing Safety Care.

 

Supervisory Responsibilities:

  • Directly supervise iChoose staff.
  • Assign work to accommodate work flow demands and mandated requirements.
  • Work closely with iChoose staff to measure program outcomes achieved by the people receiving services and coach the development of creative and engaging ways to teach new skills to people receiving services. Report information to appropriate management.
  • Document, monitor and report Staff Performance to appropriate management.

Physical Requirements and Demands:

  • Regularly required to speak and listen.
  • Regularly is required to stand, walk, sit, grasp, handle objects, and reach with hands and arms.
  • Occasionally pulls, bends, stoops, and crouches.
  • Specific vision abilities required by this job include close vision and ability to adjust focus.
  • Lifts or carries 50 pounds unassisted to assist with lifting and transferring of persons who use wheelchairs and other mobility assisted devices.

 

Tools & Equipment:

  • Multi-line telephone, PC and fax/copier.

 

Hazards:

  • Is exposed to blood and/or bodily fluids.
  • May be exposed to physical aggression.
  • May be exposed to changing weather conditions while traveling by automobile.

 

 

 

Qualifications/Requirements

Minimum Qualifications - Education and/or Experience, Certificates, Licenses, etc:

  • Bachelor's Degree in a Health and Human Services or related field OR within a year of obtaining a degree in Human Services.
  • At least a year experience working in Human Services. Preference for experience in leadership and or supervisor role.
  • Must possess and maintain current NV Driver's License and acceptable driving record (no more than 3 points, no DUI/reckless driving over past 2 years).

 

 

Language, Math and Reasoning:

  • Read, write, and speak English proficiently in accordance with essential functions.
  • Effectively present information and respond to questions from a diverse range of audiences.
  • Count, calculate figures, time, and amounts such as percentages, develop and interpret graphs.
  • Define problems, collect data, establish facts, and draw valid conclusions.

  • Ability to meet background checks requirements of the Organization.
  • Intermediate use of computer applications such as Excel, Word, and other PC based programs.
  • Tolerance of ambiguity and ability to perform essential functions with minimal supervision.

  • Ability to resolve issues and manage assigned department with a systematic approach.
  • Demonstrate flexibility when workflow or priorities change. Excellent organizational skills to appropriately manage large amounts of information.
  • High level of interpersonal skills to handle sensitive and confidential situations appropriately; includes ability to effectively present information and respond to questions from a diverse range of audiences.
