Role Summary:  Assessment Service Specialists are a primary resource for administering the iChoose assessment. They are highly interested in using data to provide information on changes for all consumers and ensure the vision of supports from Assistant QIDP's and QIDP's is implemented with high treatment integrity.

 

 

Essential Functions & Responsibilities

  • Facilitate intake process for all new iChoose program participants; approximately 1-2 per month.
    • Meet with integrated support team to complete all forms required for intake and review with Assistant QIDP
    • Set up all materials for the first day of the person served
    • Complete at least 30 assessment trials per day
    • Write up initial draft of assessment report, review with QIDP and present to support team
    • Transition person served to the service system over the course of 2nd month in program

 

 

  • Ensure all habilitation plans are making progress at an X value of 1.2 or above and that habilitation plans are implemented with fidelity.
    • Ensure habilitation plans are monitored for progress
    • Flag habilitation plans for QIDP review
    • Create and implement training plans for staff for habilitation plans in coordination with QIDP
    • Ensure all materials for habilitation plans are ready and easy to access

 

 

  • Ensure all iChoose assessments are implemented in a timely manner while providing life, health and safety supports to a ratio on the service floor.
    • Collect data on treatment fidelity of assessment and present to QIDP
    • Make adjustments to assessments implementation steps in coordination with QIDP
    • Develop communication plans for all levels of the support team in coordination with QIDP; utilize communication plans and report results to Day Services Management Team
    • Record training videos for assessment implementation

 

 

  • Data Collection
    • Assist QIDP in implementing all data collection systems
    • Review all data collection systems and report to QIDP
    • Create trainings for data collection systems and adjust based on training outcome in coordination with QIDP
    • Provide feedback to habilitation plan specialists using BST model
    • Administer feedback cards and present data to create changes in card system in coordination with the QIDP

 

  • One on one services
    • Ensure QA for 1:1 services are completed for all cases
    • Monitor incident reports for all 1:1 services
    • Attend team meetings for 1:1 services
    • Provide feedback to habilitation plan specialists using BST model

 

  • Additional Responsibilities
    • Work closely with Day Services Managers to measure program outcomes achieved by the people receiving services
    • Participate in training and learning opportunities. Introduce new techniques and/or information learned to improve knowledge and processes in the department
    • Identify opportunities and take action to build strategic relationships between one's work area and other areas, teams, departments, etc. to achieve business goals.
    • Work in a team environment and contribute to the team's success by making suggestions, helping co-workers, participating in team activities, collaborating with other departments and acknowledging individual and team accomplishments.

 

 

 

Qualifications/Requirements:

Minimum Education and/or Experience:

  • High School diploma or equivalent required
  • Preference will be given to individuals with experience working in a capacity involving data collection under clinical behavioral supervision in teaching or social service environments
  • Preference will be given to those with completed coursework in psychology, social work, human development or a related field that would demonstrate mastery of a knowledge base that includes: understanding developmental disabilities, therapeutic interventions, conflict prevention and response, psychopathology and developmental disabilities, teaching and active treatment, and assaultive behavior interventions
  • Preference will be given to individuals with experience in standard celeration charting and precision teaching

 

Ability to:

  • Make data based decisions and demonstrate commitment to a data oriented approach to service delivery
  • Implement a free operant based training program with minimal guidance
  • Demonstrate fluency in response blocking, three tier prompting, pivoting, providing rein, setting aims and using goal setting to achieve outcomes with people served.
  • Demonstrate systems level orientation to habilitation plans
  • Demonstrate excellent crisis intervention skills and the ability to identify behavioral functions
  • Learn and apply training to carry out job duties and comply with policy
  • Demonstrate patience, tenacity and creativity while providing direction to people served and staff
  • Demonstrate good judgment and work independently with little or no supervision
  • Recognize priorities and organize time appropriately to meet departmental needs
  • Maintain a high level of professionalism including respecting the rights of people receiving services, maintaining appropriate boundaries, and behaving appropriately with all levels of customers (individuals served, co-workers, customers, residents, family members, agency staff and supervisors)
  • Communicate with diverse groups of people (e.g. all levels of customers, coworkers, and the general public) in a kind, professional and contextually appropriate manner including the ability to positively represent HSI/WARC and effectively describe programs and services offered
  • Meet HSI/WARC criminal background check and drug testing requirements
